This position is primarily located in an office setting in Seattle, with the work occurring via a hybrid model of time in the office and time working remotely from home. Job Summary
The East King County System Navigation Hub is a regional program funded by the Veterans Seniors and Human Services Levy (VSHSL) that provides person-centered services and resources to low-income seniors and adults with disabilities and their caregivers through a network of community partners and service providers to support stabilization in the community and increase quality of life.
Job Description
The Program Coordinator is responsible for program administrative and operational support which includes recruitment and maintenance of hub partner relationships and the oversight of intake and hub provider referral and service plan development for hub service recipients. The work requires the ability to communicate clearly both orally and in writing, maintain professional boundaries, and effective time management with varying day-to-day activities and related workflow. The position reports to the Project Manager. This is a non-exempt, union, full-time, regular position.
Essential Duties:
Networking
- Establish collaborative relationships with hub partners including community-based social service providers, community centers, health care providers, advocacy organizations, and other stakeholders as universities and government entities.
- Actively collaborate ongoing with hub partners to link hub service recipients, establish shared care plans, organize ongoing multi-disciplinary team meetings, and organize periodic training and resource sharing events, attend community meetings, etc.
Administrative and Data Reporting
- Facilitate tracking of hub service recipient intake, referrals, and hub service linkages in the database. Process incoming and outgoing mail.
- Assist with scheduling trainings for Lifelong programs as needed. May assist with other project work and duties as assigned.
- Help maintain documentation and accuracy of HART high risk list.
- Review grant opportunities and edit drafts
Client Service
- Conduct hub service recipient intake and resultant hub partner referrals and service linkages.
- Provide HART program support, by individualized care plan support to hub service recipients and their caregivers. Coordinate with a variety of health care and social service providers and community partners to respond to established shared care plan.
- Document provided services and assure shared care plans are documented and distributed to appropriate hub partners. Maintain strict confidentiality of client records.
- Coordinate with Caregiver Case Manager to provide wrap around supports for long-term and sustainable client care
Quality Assurance
- Assist with the development and disbursement of surveys.
- Perform routine maintenance of client documentation and related correspondence. Review Hub goals and objectives to ensure that program is reaching milestones
- Distribute annual service linkage reports to Hub partners
Communication
- Communicate with all program staff, interagency staff, and community partners and providers as needed. Communicate with hub service recipients and their caregivers including other formal or informal supports to perform duties of the position.
. Education and Experience:
- Bachelors degree in human or social services, public administration, or related field preferred.
- Individuals with lived experiences in navigating social services are encouraged to apply.
- Experience working with the disability community preferred.
- Direct human or social service experience required; established experience with East King County social services and related system navigation preferred.
Core Competencies:
- Familiarity with individuals who experience health disparities, including complex chronic health conditions, behavioral health barriers, chemical dependency, poverty, and stigma-induced isolation.
- Comfort with working with individuals along the entire continuum of diversity, including but not limited to varying abilities, race, class, gender identity, and sexual orientation.
- Able to organize, prioritize multiple projects, and meet deadlines in a time-sensitive environment.
- Demonstrated skills and strong working knowledge of Microsoft Office suite of applications, including Excel, Outlook, Word.
- Ability to communicate clearly and professionally in both verbal and written form.
- Proven ability to work independently and in a collaborative team environment.
- Ability to develop good working relationships with internal colleagues and external clients
- Ability to understand and model the mission and ethics of Lifelong and the Aging and Disabilities department.
Work Environment:
- Primarily remote with intermittent office visits and community based outreach to partners and clients in East King County. If safe, once a month in-person Hub staff meeting.
- All staff are currently required to use personal protective equipment to prevent exposure to COVID-19, and proof of full COVID-19 vaccination is required upon hiring.
All qualified candidates must possess a valid drivers license and have access to a vehicle each work day. Mileage will be paid at the current Internal Revenue Service rate when staff is assigned to conduct business in their own vehicle.
