Vice President of Value-Based Care

National Health Care Associates, provider of high-quality long-term care and short-term rehabilitation services with skilled nursing facilities in the Northeast U.S., is seeking a Vice President of Value-Based Care to join our Executive team! This is a fantastic opportunity for the right candidate to join one of the premier healthcare organizations in the Northeast. The VP will travel to our facilities in the New England region (including Maine, Connecticut, Vermont and New Hampshire) and New York as needed, with roughly 50% of the time spent on the road and the remainder in a home office and/or our Wethersfield, CT offices.
Sampling of Position Responsibilities:
- Direct and participate in the design, development, and implementation of value-based care delivery models to improve adherence to evidence-based practices, improve quality outcomes.
- Apply your expertise in Value-Based Care including Bundled Payment Programs.
- Partner with Business Development Groups internally and our external partners.
- Work proactively with our skilled nursing facilities and regional clinical consultants, ensuring they are current and quality metrics and clinical outcomes are being measured appropriately and actioned upon.
- Develop and nurture relationships both new and ongoing with ACO's (Accountable Care Organizations)
- Clinical program development and maintenance.
- Ensuring education and program standards are utilizing best practices.
- Develop, implement and assure reporting mechanisms are in place to monitor processes, patient care, outcomes, and safe and effective disposition in value-based care delivery arrangements. Collect and analyze data on clinical indicators to identify and communicate opportunities for improvement.
- Direct supervision of the Clinical Integration Specialists Team.
- Organize and integrate multiple organizational priorities and deadlines across departments.
- Work with our centers to reduce rehospitalization rates
- Collaborates with Senior Level Clinical and Operational Leadership to forward our mission, value, and goals.
What You Will Need:
· Track record of success as an RN, PA, MD or other clinical background in a Skilled Nursing environment.
· A minimum of 8 years of experience in Senior Level management (Preferred)
· Ability to travel to our facilities throughout New England (including Maine, Connecticut, Vermont and New Hampshire) and New York when necessary

Pay: $200,000.00 - $225,000.00 per year
