Excellent In-Home Care is looking for experienced RN (Registered Nurses) to join our dedicated team. The candidates must have a minimum of 1 year of professional home Health nursing experience. Our service area includes Los Angeles County, Thousand Oaks, and Westlake Village We use Axxess for EMR. The position is to cover Los Angeles County and surrounding areas. The candidate will set preferred areas to cover.

As a Visiting RN (Registered Nurses) you will provide in-home care for adult or Geriatrics patients at patients home/Assisted Living/Retirement homes.

CANDIDATES MUST HAVE:

  • Current active RN license (Registered Nurse) in the state of California
  • Current CPR,Health clearance, TB clearance
  • Minimum of one year of professional Home Health nursing experience
  • Must pass a criminal background check
  • Driver’s license
  • Proficiency in EMR charting and timely submission of documents
  • Proficiency in wound care, catheter insertion, diabetic care, vital signs monitoring, ostomy care

DUTIES OF VISITING REGISTERED NURSE INCLUDE:

  • Completing an initial home health assessment (SOC, ROC, RECERT& OASIS DC , FOLLOW-UP)of the patient and family to determine home care needs.
  • Providing a complete physical assessment and history of current and previous illness.
  • Conducting home visits as assigned, assessing the patient's condition, progress toward goals, and providing patient care, teaching and other skilled interventions as ordered.
  • Regularly re-evaluating patient nursing needs, as part of nursing visit.
  • Initiating appropriate preventive and rehabilitative nursing procedures.
  • Administering medications and treatments as prescribed by the physician.
  • Providing health care instructions to the patient as appropriate per assessment and plan of care.
  • Communicating needs to the team as part of the care plan development; assists in implementing prior to discharge of the patient.
  • Informs personnel of changes in the condition and needs of the patient.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with Agency defined time frames.
  • Assuring proper maintenance of clinical records in compliance with local, state and federal laws.

Excellent In-Home Care, Inc. works on a referral basis. Though referrals typically originate with the physician, they may originate from any local hospital, doctor's office, board and care, skilled nursing facility, patient, family or interested person or agency.

Once a nurse or a clinical representative of Excellent In-Home Care, Inc. receives the referral information, the patient will be evaluated within 48 hours or on the date specifically ordered, a professional clinician or team of clinicians will determine the proper plan of care and will begin to administer that care.

All our nurses are eminently qualified to carry out a complete range of clinical duties in a most caring and professional manner.
