Wraparound Service Provider
Job description
Job Summary: The Wraparound Service Provider is responsible to provide socially necessary services to children and families through the Safe-At- Home Program and with children and families enrolled in the severe emotional disturbance waiver program. Services include but are not limited to: crisis support, supervision, parenting education, life skills, child -oriented activities, tutoring and transportation assistance. Documentation, summaries and/or reports of services are required. Transportation to and from consumer homes, schools and other social service and community agencies is necessary. This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices.
Duties and Responsibilities:
- Provides in-home supportive services including: parenting education, supportive counselling, community resource referral, advocacy and other social services.
- Maintain a schedule for each youth/family referral to ensure that services occur as defined in program manuals and individual Plan of Care.
- Participate in the Wraparound Team Meeting and Plan development.
- Establishes and maintains strong relationship with assigned families.
- Document interactions between child and family.
- Maintain an active record of all services provided.
- Attend Multi-disciplinary Planning Meetings and court hearing as required.
- Provide monthly report for Facilitator.
- Provide transportation as required.
- Employees are required to use their personal vehicles in certain circumstances and as directed in the performance of their job duties.
- Monitor and evaluate service delivery and performance measures.
- Provide linkage and referral to meet identified needs.
- Assist with helping client develop a natural support system.
- Perform other duties as assigned.
Required Skills/Abilities:
- Willingness ability to travel throughout the State
- Use of vehicle to transport clients required.
- Basic knowledge of and commitment to children who have experienced trauma and/or display serious emotional/behavioral health issues.
- Ability to complete Agency required training, Wraparound and WV Safe at Home training
- Ability to provide stabilization resources for crisis situations
- Ability to work independently while adhering to a team environment.
- Strong interpersonal skills.
- Good written and oral communication skills
- Ability to use Adobe and Microsoft software
Education and Experience:
Requires Bachelor Degree in Social Work or human services related field or 4-6 years’ experience working with at-risk youth and families. Certified to administer Child and Adolescent Needs and Strengths. Must have valid driver’s license and proof of automobile insurance as required by law
