The Health Home Coordinator could be the perfect position for you!
The Health Home Coordinator for Members with Serious and Persistent Mental Illness (SPMI) will be responsible for the following:

• Write the Plan of Care for each member.
• Oversee the plan and coordination of services.
• Identify and link member with community resources.
• Facilitate referrals and respond to social service needs.
• Collaborate with other team members including doctors, nurses, other care coordinators, and clinicians.
• Help members implement the Plan of Care.
• Monitor and assist them in problem solving potential issues related to the health care system, financial or social barriers.
• This roll will also support and encourage members in actively participating in reaching the goals they set together!

MHRT/C Required! ABHS keeps up on your required continued education hours for you!

Job Type: Full-time

Pay: $20.00 - $25.00 per hour
