Under the general supervision of the Program Manager, Case Management Services, and under the general policies and procedures this position is a member of the Ryan White Case Management team. The Case Manager, AIDS Services Non-Medical assesses the needs of clients with HIV/AIDS and connects them with internal and/or community resources; assists clients with accessing public/private benefits and entitlement programs; coordinates with staff and other service providers to get client needs met. Community outreach is integral to this role.

Assess each client’s current situation and needs, as well as what services they are currently utilizing, in order to refer or assist appropriately.

Determine eligibility and educate clients on all public and private benefit programs such as Ryan White funded services, Aids Drug Assistance Program (ADAP), Medi-Cal, Social Security, State Disability, Unemployment, General Assistance, Veterans Benefits, etc.

Assist clients retain benefits and provide advocacy as needed.

Develop a comprehensive, individualized care plan and coordinate the services required to implement the plan.

Monitor clients to assess the efficacy of the care plan.

Periodically evaluate the plan and adapt changes as necessary.

Manage referrals, linkages & outcomes including proper documentation.

Complete and submit client requests for emergency financial assistance, transportation and exceptions.

Provide outreach services to potential clients and the community at various locations, including, but not limited to, testing locations and health fairs.

Participate in ongoing education and training, remain up-to-date on regulations, eligibility requirements, programs changes, and new benefit offerings.

Collaborate with multidisciplinary teams.

Maintains all HIPAA standards of privacy and confidentiality.
Other duties as assigned.

Bachelor’s degree in Health or Human Services or related field or equivalent combination of training and experience.

One year of case management experience

Bilingual Required; Spanish or Vietnamese preferred.
PREFERRED EXPERIENCE & TRAINING
Spanish or Vietnamese language proficiency
Case management experience with HIV positive persons, and/or persons with a history of mental illness, homelessness, chemical dependence and experience with families.

Knowledge of community resources.

Knowledge and experience with public/private benefits and entitlement programs.

Knowledge of HIV/AIDS. Adept at establishing rapport with clients, families, health care professionals, and community service organizations.

Ability to establish and maintain appropriate professional boundaries, and to maintain a high standard of professional and ethical conduct.

Ability to work independently, prioritize, and make effective use of time.

Ability to efficiently handle demanding workload involving multiple tasks, and master detailed and complex information.

Ability to handle conflict, crisis situations, and difficult clients.

Excellent written and oral communication skills.

Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner.

Demonstrated proficiency in Microsoft Suite and Google products

Self-motivated and demonstrated critical thinking skill

Demonstrated strong teamwork and people skills, with ability to develop productive relationships

Normal office environment with little exposure to excessive noise, dust, and temperature. -
Local travel required.

Car available for daily use with valid driver’s license and automobile insurance as required by the State of California.

Duties are performed in a combination of the office setting and the field.

Salary Range:
$29.25 to $33.25/hr
