Position Title: Patient Engagement Program Manager
Status: Full-Time, non-exempt
Supervisor: Administrative Director
Hours: 8:30 AM - 5:00 PM; Some evenings and weekends may be required.
Work Location: Primary office is at Henrico clinic; work at Richmond Clinic also required. Travel between clinics is expected on a regular basis.

About CrossOver Healthcare Ministry
CrossOver Healthcare Ministry provides compassionate, high-quality healthcare to the uninsured and medically underserved in our local community through the operation of two healthcare clinics; one in the City of Richmond and one in Henrico County. CrossOver is a 501(c)(3) non-profit organization that offers comprehensive healthcare services to its patients, including: primary care, specialty care, pediatrics, dental, eye care, mental health counseling, HIV/AIDS care, OB/GYN, medications and medications management, and case management. Position Summary
Under the general supervision of the Administrative Director, the Patient Engagement Program Manager will be responsible for ensuring that all patients at CrossOver have a high level of satisfaction with services while engaged in care at CrossOver. Area of focus includes all services offered by CrossOver (including but not limited to primary care, pediatrics, pharmacy, OB, eye, behavioral health, social work, dental, hospital discharge, and refugee screening). Also includes non-clinical services such as patient intake and patient renewals. This position oversees the patient feedback process and develops strategies to improve results.
Key Responsibilities
Patient Eligibility, Intake and Renewals
- Ensure that the patient intake process and renewal process is patient centered, accurate, and efficient;
- Partner with the Director of Clinical Operations to determine appropriate number of patients to be admitted into the practice;
- Maintain policy and procedure manual for the patient intake process and renewal;
- Ensure training for financial screening employees and volunteers is performed annually;
- Ensure timely and accurate submission of AccessNow referrals;
- Collaborate with Medications Program Manager to ensure that financial screening process is compliant with RxPartnership regulations and assist with audit preparation.
Medicaid Coordination
- Ensure that Medicaid eligible patients are identified and enrolled in Medicaid as quickly as possible;
- Identify patient whose Medicaid has lapsed and offer assistance and/or resources with re-enrollment;
- Serve as employee resource for Managed Care Organizations, Medicaid updates and other Medicaid related topics to all departments;
- Maintain policy and procedure manual for Medicaid;
- Collaborate with Operations Specialist to develop strategies to maximize Medicaid revenue;
- Serve as Organizational Expert on resources and service available for Medicaid patient through the MCOs.
Patient Engagement
- Provide overall project coordination for patient experience and engagement analysis;
- Facilitate organizational-wide consistency and effectiveness in applying, understanding, and using patient satisfaction/ experience data results;
- Identify areas of improvement for patients' experience and work in cross-functional teams to provide accurate and focused feedback for improvement;
- Oversee and perform quality assurance processes for patient engagement, and all patient intake processes;
- Works with administration and clinic staff to ensure quality patient care and that services are delivered inclusively and equitably;
- Partnering with CrossOver’s Outcome Analyst, develop a Patient Engagement Dashboard to measure progress.
Community Engagement
- Organize and participate in outreach events to promote CrossOver and establish community partnerships;
- Work collaboratively with partners (hospitals, health departments, schools, partners) to establish pathways to refer patients into CrossOver;
- Assist in the creation and design of patient outreach literature.
Volunteer Management
- Collaborate with the Volunteer Program Manager to identify ways that volunteers can be integrated into the Case Management Program;
- Create a welcoming, supportive environment for volunteers;
- Train, mentor and supervise volunteers.

Skills and Qualifications
- Familiarity with Medicaid and non-Medicaid community resources in the greater Richmond Area;
- Familiarity with Access Now requirements;
- Must be able to apply proficient knowledge of Virginia state assistance programs (Medicaid);
- Strong understanding of survey design, methodology, and data analysis for decision-making;
- Exceptional oral, written, and presentational skills;
- Ability to work in cross-functional, diverse teams;
- Must be bilingual English/Spanish;
- Bachelor’s degree in related discipline preferred.
