Overview:
The Health Coach provides guidance, support and resources to clients and primary family caregiver on the CCSP/SOURCE Structured Family Caregiver program (SFC). The Health Coach in collaboration with the branch RN provides an assessment of the needs of the SFC client and creates a “coaching plan”. The Health Coach monitors and provides ongoing assessments, training and support to the primary (family) caregiver and the client adjusting as necessary.
External Job Description:
- Provides assessment with collaboration of the RN to ensure the SFC client goals and needs are being met.
- Prepares a “coaching plan” for the Primary Family Caregiver to follow to provide ADL and IADL care for the SFC client.
- Assist the client and the Primary Family Caregiver with determining eligibility for assistance and resources to compliment the SFC program.
- Provides coaching and training to the Primary Family Caregiver to ensure all goals of the client are being met.
- Maintains effective communication with the Primary Family Caregiver, RN, and Medicaid Waiver Case Manager.
- Possesses knowledge of community resources related to medical issues, housing issues, financial and social issues for dealing with client’s care. Makes referrals to other professionals and programs.
- Monitors the Primary Family Caregiver’s daily progress notes and tasks performed via the electronic visit verification application.
- Conducts monthly contact visits with the client and the Primary Family Caregiver by home visit or telephone.
- Schedules case conference with the client, the Primary Family Caregiver, the RN and the Medicaid Waiver Case Manager on a semi-annual basis.
- Sends summary reports to the Medicaid Waiver Case Manager until the external portal is developed on a monthly, quarterly and as needed basis.
- Must has two years of experience working with elderly, medical frail and disabled clients.
- Must be an LPN with current GA license.
- Must have excellent communication, computer and organizational skills.
- Knowledge of GA Medicaid Waiver programs and home and community bases resources a plus.
