LICENSED CLINICAL SOCIAL WORKER JOB SUMMARY

The Medical Social Worker is responsible for the delivery of varied social work services to the patient and family. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, bereavement services for Hospice, community education, outreach and referral. The Medical Social Worker also acts as a consultant to the agency staff to help them better understand the significant social and emotional factors concerning the patient.

LICENSED CLINICAL SOCIAL WORKER BENEFITS

LICENSED CLINICAL SOCIAL WORKER ESSENTIAL FUNCTIONS

-Assesses the social and emotional factors in order to estimate the patient and family capacity and potential to cope with problems

-Helps the patient and his family to understand, accept and follow medical recommendations

-Observes, evaluates, and brings to team conferences information regarding psychosocial, spiritual, physical, and financial conditions affecting the patient and family

-Develops and maintains contact with appropriate community agencies and services in order to promote inter-agency cooperation and to facilitate related referrals

-Assumes the active role of advocate for the patient/family unit

LICENSED CLINICAL SOCIAL WORKER QUALIFICATIONS AND EXPERIENCE

-Masters degree in Social Work

-Demonstrated ability in casework, counseling and group work, including two (2) years social work experience in a hospital, hospice agency, out-patient clinic, home health agency, medical rehabilitation, medical care or mental health program.

-Valid license to provide services as LCSW or CSW in the State of Utah is required.

-Valid Driver’s license, car insurance and car required.
