Position Description: The LPN Care Leader position will coordinate all resident services under the supervision of the RN Wellness Director and Assisted Living Wellness Director. The Care Leader will maintain a continuing awareness of each resident’s total wellbeing (social, family, financial, mental health, key life events, dietary needs, spiritual needs, health, and medical needs; including, but not limited to medications and personal care needs) featuring Hearth Management’s Live More programming and Think More programming philosophies.
Education/Training/ Experience: A valid NY LPN state license with (3) years full-time experience in the provision of services to a frail, dependent, adult population. Knowledge of geriatric nursing and dementia nursing
