Medication Safety Pharmacist


The Medication Safety Pharmacist is responsible for the management and coordination of safe medication practice to ensure the safety, quality, and effectiveness of medication use throughout the pharmacy department and hospital. The medication safety pharmacist will monitor and manage medication safety metrics which serve to evaluate pharmacy and hospital medication workflows in order to identify and mitigate high-risk medication practices. The medication safety pharmacist will be responsible for conducting safety assessments and medication error/adverse drug event analysis to improve patient outcomes. They will be responsible for reporting safety metrics to the Medication Safety Committee which serves to provide effective communication among departments regarding safe medication practice, consistent with the hospital’s mission to provide high-quality patient care.

Qualifications:
  • Graduate of an accredited school of Pharmacy required.
  • Minimum five years of Hospital Pharmacy experience in an organized health setting required.
  • Demonstrated Leadership ability required.
  • Licensed as a Pharmacist in VA required.
  • Medication Safety Certification to be completed within 12 months of employment.
