Responsibilities:

River Vista Behavioral Health sits along the bluffs overlooking the San Joaquin River, this brand new, 128-bed facility, will provide high-quality and advanced behavioral health services to residents and visitors in the Central Valley. The new hospital will employ more than 250 people, including clinicians, nurses, mental health technicians, support staff and administration.


We are currently seeking a compassionate and hardworking ASSESSMENT & REFERRAL MANAGER. The Assessment & Referral Manager is responsible for the daily management of the Assessment and Referral activities at the Hospital. As a clinician, this position provides supervision to a staff of Assessment & Referral Clinicians in the provision of psychiatric assessment services, including clinical evaluations, crisis intervention, and referrals. This position interfaces closely with the medical staff, administration, treatment team, external case managers and managed care organizations, and functions as a liaison with other hospital intake departments. The A&R Manager is the department supervisor performing department administration and supervision and also performs psychiatric assessments for incoming new patients, and as requested for continuing patients.

QUALIFICATIONS

Education:
Master’s Degree from an accredited program in nursing, social work, mental health or related field preferred.

Experience:
A minimum of three (5) years direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations.

Licensure:
R.N., L.C.S.W., L.V.N., L.P.T. or applicable licensure or Master’s level and license eligible.

Additional Requirements:
Complete knowledge of crisis intervention techniques. Trained in Handle with Care and Cardiopulmonary Resuscitation

PRIMARY RESPONSIBILITIES

Department Administration

  • Prepares and monitors ongoing department staffing and schedules to meet department specific requirements and ensures proper coverage is maintained either by covering additional shifts or contacting team members to cover unanticipated staffing shortages.
  • Prepares and reports daily to the Clinical Operations meeting all admissions within the prior 24 hours.
  • Responsible for verification of all staff time sheets, obtaining employee signatures and forwarding time sheets to payroll in a timely manner.
  • Review of all shift admissions to ensure appropriate authorizations have been obtained.
  • Prepare Performance Improvement and Department end of month reports based on data collect of specified criteria by Administration and the Performance Improvement Council. Responsible for consistent and timely audits of COBRA logs and ensuring all collected data is reported monthly.
  • Assure adequate orientation for new employees of the Assessment and Referral Department.
  • Ensure an adequate system of communication and reporting is maintained between all staff members, as well as other departments involved in the ongoing operation of the program.

Patient Assessment
  • Participate in screening patients for admission into hospital programs, utilizing pre-approved admissions criteria.
  • Provide assessments of incoming patient’s mental health/psychiatric condition patients through various methods.
  • Prepare written reports of assessment results and prepares recommendations for the level of care and treatment needs.

Treatment Planning
  • Evaluate patients’ needs and advocate the lowest level of care consistent with the defined patient needs.
  • Interface with the facility staff that will provide the appropriate level of care, to ensure a smooth transition for the patient.
  • Promote an effective use of resources for patients, customers and the facility with sensitivity to cost of health care.
  • Observe patient to detect behavioral patterns and report observations to medical staff.
  • Assist with completing initial admissions forms for patients as required.
  • Intervene to restrain violent or potentially violent or suicidal patients by verbal or physical means as required.
  • Contact patient’s relatives by phone to arrange family conferences as required in a timely fashion.
  • Begin patient/family education at time of admission.
  • Provide information about the patient to the program staff who will be receiving the patient.

Qualifications:
Specific Standards for the Care of Adolescent Patients Ages 12 through 17
  • Maintain knowledge of the Adolescent Program rules, protocols and handbooks guidelines.
  • Assess patients for behaviors consistent with the developmental stages and makes note of any deviations in behavior.
  • Recognize acting out behavior of adolescent specific age group and set appropriate limits in clear, concise, behavioral and non-judgmental terms based on the age of the patient.
  • Demonstrate an awareness of learning disabilities and translate treatment/program expectations to the learning disabled adolescent.
  • Relate to adolescent patients in an age appropriate manner.
  • Utilize seclusion and restraint procedures with an understanding of the need for modification in regard to smaller body sizes of adolescents, abandonment issues and the history of physical and/or sexual abuse issue.
  • Act as a resource to other staff in regard to the Adolescent Patient Programs.

Specific Standards for the Care of Adult Patients Ages 18 through 624
  • Gather information from the patient, family and interdisciplinary team and use judgments based on knowledge of adult behaviors in treatment planning and interventions.
  • Recognize behaviors and set appropriate limits in clear, concise, behaviors and non-judgmental terms.
  • Utilize seclusion/restraint procedures applicable to adult body size and past history of violent or emotional behaviors.
  • Maintain current knowledge of the Adult Program rules, protocols and handbook guidelines.
  • Utilize theoretical concepts to guide the effective practice of adult care within Specific Standards of Care for this age group.

Specific Standards for the Care of Geriatric Patients Ages 63 and older
  • Maintain knowledge of the developmental tasks and sociological theories of aging.
  • Utilize theoretical concepts to guide the effective practice of gerontological health care and behavioral care.
  • Gather the appropriate information from the patient, family and interdisciplinary team and use therapeutic judgments based on knowledge of gerontological health/behavioral care practices to develop a comprehensive plan of care.
  • Utilize interventions based on gerontological theory to restore patient’s functional capabilities and to prevent complications and excess disability.
  • Maintain a complete knowledge of treatments, medications and side effects of such in the geriatric age group.

Specific Standards for the Care of Chemically Dependent Patients
  • Assesses patient needs as related to chemical dependency and document the plan of care from the admission through discharge.
  • Collaborate with other chemical dependency staff professionals in planning for discharge and follow-up care.
  • Maintain an awareness of how personal issues relating the co-dependency can impact work performance/relationships.
  • Recognize and establish limits on medication-seeking behaviors.

  • Attend all mandatory facility in-services and staff development activities as scheduled.
  • Adhere to facility standards concerning conduct, dress, attendance and punctuality.
  • Support facility-wide quality/performance improvement goals and objectives.
  • Maintain confidentiality of facility employees and patient information.


PHYSICAL, MENTAL, AND SPECIAL DEMANDS

  • Ability to stand or walk up to six hours, sit for up to two hours.

  • Ability to occasionally push/pull wheeled carts from 200 lbs. (i.e. crash carts, patient on stretchers or in wheelchairs).

  • Ability to handle hazardous/infectious waste on an occasional basis.

  • Ability to respond to exposure to blood and body fluids.
