Social Service Caseworker

Social Service Caseworker Responsibility Highlights include:

• Meeting with residents and/or responsible parties to review policies on advanced directives, living wills, DNR status, residents’ rights and discharge planning. Implement documents as appropriate with residents and/or their responsible parties.

• Conduct and document social service evaluations including identification of residents needs/issues and arrange for appropriate social support services.

• Provide assistance to residents and/or their responsible parties in obtaining financial services, as appropriate.

• Ensure that social services are coordinated with the other facility services.

Minimum Requirements

• Bachelor's degree from an accredited college or university in social work or a related field and two (2) or more years experience in providing social services in a health care setting.

• Excellent computer skills in Microsoft Windows
