Boston Children?s Health Physicians The Maria Fareri Children?s Hospital @ Westchester Medical Center has an opening for a Social Worker within the division of Pediatric Hematology/Oncology/Stem Cell Transplant. The Social Worker will be part of a multidisciplinary team of medical professionals and a growing psycho-social team including Psychologists and Child Life Specialists, treating children and young adults with a hematology/oncology diagnosis. Responsibilities include completing a psycho-social assessment at diagnosis and management of social determinants of health for patients, and their family members in both the in-patient and out-patient setting. Psychosocial counseling will be provided to patients and families regarding the impact and adjustment to illness, compliance to treatment plan, crisis intervention, bereavement, and quality of life and survivorship. Participation in interdisciplinary rounds, as well as frequent communication, collaboration and outreach to community organizations is expected. Maintain timely hospital documentation as required by the CMS Conditions of Participation for all in-patients. Salary range approximately $75,000/year-$80,000/year, negotiable based on experience.

Qualifications:

• Social Worker LCSW is preferred but will consider MSW with experience in the health care setting and ability to sit for LCSW exam within a year of hire
• Licensed or License eligible in NYS
• Spanish speaking providers encouraged to apply
