Dade Family Counseling CMHC is a Community Mental Health Center licensed by the State of Florida and accredited by The Joint Commission. Founded in 2003, we operate a full service Community Mental Health Center that treats children, adolescents and adults with mental illness, substance abuse issues and/or co-occurring disorders. We accept most insurance plans and work with most private and public insurance providers.

We are a staff of approximately 65 clinical and non-clinical personnel which includes Psychiatrists, ARNPs, LPNs, Licensed Mental Health Clinicians, Counselors and Targeted Case Managers. We have the following patient programs and services available: PSR (Psychosocial Rehabilitation Program), TCM (Targeted Case Management) for adults and children, Group Psychotherapy, SAP (Substance Abuse Program), Individual/Family/Couples Counseling, and Psychiatry/Medication Management.

Position: Targeted Case Manager

Locations: Miami, FL

Full Time Salary: $45K Base Salary. Up to $55K or more with bonus billable hours.

Mileage: $200 mileage stipend per month

Requirements: 40 Billable hours/week (Opportunity to obtain a generous stipend bonus for any billable hours over expectation).

Equipment Provided: Laptop & Cell Phone

Part Time Hourly: $21.63/Hr

Requirements: Based on the number of hours worked, 95% of the total hours works must be billable hours.

Mileage: $50 -100 mileage stipend per month depending on caseload and scheduled hours.

Major tasks include but are not limited to:

  • Providing linkage, coordination and monitoring of needed mental health and medical services to adults, adolescents, and children who are mentally ill, are unable to advocate or coordinate for themselves, and lack the support system to assist them in accomplishing these tasks.
  • Providing clients with home assessments and on-site services at community agencies, schools, courts, or any other location where the client may need advocacy or assistance.
  • Maintaining case records in a timely manner. Documenting client progress and interventions in accordance with HIPAA guidelines and other federal and state regulations.
  • Effectively communicating with the courts and providing documentation when needed for Jail Diversion Program clients.

Minor tasks include but are not limited to:

  • Monitoring the health and safety of clients.
  • Assuring the rights of clients are being abided by at all times.
  • Collaborating with police officers, probation/parole officers, school officials, employers, and other treatment providers as requested by the client.
  • Participating in team/department meetings and individual supervision sessions to discuss clients.

The ideal candidate will have the following qualifications:

  • Bachelor's degree in social work, psychology, or another social service related field.
  • Minimum 2 years experience in social services or related field.
  • Current TCM certification or willing to attend 1 full-week training program within 90 days of employment.
  • Fluent bilingual English & Spanish (Required)
  • Excellent organizational and written/verbal communication skills.
